Złoty Liść can help with a ZUS funeral benefit application and direct settlement in eligible cases. The amount credited and any remaining payment are agreed as part of your funeral arrangements.
Check the benefit that applies
ZUS is Poland's Social Insurance Institution. For deaths from 1 January 2026, the family funeral benefit rate is PLN 7,000, subject to eligibility and the rules for shared costs. Other claimants may receive a smaller amount.
The benefit amount is not a package price or confirmation that every funeral will be fully covered. Tell us about the relevant insurance or pension status and who is meeting the funeral costs.
Agree the complete bill and payment plan
- Our services and their agreed charges.
- Any cemetery, parish or other external charges.
- The benefit amount expected to be credited.
- Any advance payment or remaining balance, with due dates.
- What happens if ZUS delays, reduces or refuses the award.
Ask for these points in writing before authorising services. If an insurer or another relative is contributing, keep that contribution separate from the ZUS benefit.
Authorise the application and settlement
The Z-12 application can authorise payment of all or part of the benefit to the funeral home's account. We can explain the documents required and how that payment will be applied to the invoice.
Keep copies of the agreed estimate, authority and claim documents. ZUS decides entitlement; submitting an application does not itself confirm an award.
If you are arranging from abroad, ask which signatures and originals are needed before sending anything.
